Approval of Contact Hours for Continuing Education

Individuals, organizations or agencies desiring approval of contact hours for a continuing education activity can utilize this form to submit their payment.

Fee Overview:

• $200 Application Fee (Includes one contact hour)
• $40 for each additional contact hour being requested
  (Example: Three contact hours requested- $200 + $80 = $280 fee)

Note: Fees are not prorated for less than one contact hour. If requesting a portion of a contact hour, round up to the nearest whole. Example: Fee for 1.5 contact hours = $240 (calculate for 2.0)
